Relevant personal data is collected and stored in order to deliver speech and language assessment and intervention for children. Clients' written case notes, and in some cases audio / video recordings, will be kept on file. Case notes will include personal identity information such as name, address, and contact details. Electronic personal data will be kept on an encrypted, password protected laptop and any accompanying paper notes will be stored in a locked facility until they can be transferred to the client’s electronic file.
It is sometimes necessary to share personal information about children with other healthcare professionals and educational staff. We will always ask consent before sharing information about a child unless obliged by law to do so.
All clients have the right to access and request to amend the personal information stored by Lowri Speech Therapy.
We are obliged by law to hold clients’ records until they reach 26 years old - after this time they will be destroyed.
It may be necessary to make changes to this policy in response to Data Protection laws and guidance issued by the ICO. This policy was last updated in March 2021.
